Evening Chronotype Is a Key Vulnerability Factor for Shift Workers’ Diet and Mental Health

Roughly one in five workers in industrialized economies now works some form of rotating or night schedule, a pattern that fundamentally clashes with the body’s internal circadian clock. The health consequences of shift work have been well documented, from elevated cardiovascular risk to metabolic syndrome. But not everyone who works shifts pays the same price. A new systematic review published in Chronobiology International pinpoints a specific vulnerability factor: the evening chronotype.

The review, led by Franky Giltelin Emmanuel Ngelayang and colleagues at Universiti Teknologi MARA in Malaysia, followed PRISMA 2020 guidelines and searched four databases from inception through April 2026. After screening, 29 studies met the inclusion criteria, covering primarily nurses and healthcare workers with additional representation from manufacturing and service sectors. The team assessed study quality using the Joanna Briggs Institute Critical Appraisal Tools and synthesized findings on two distinct but interconnected domains: eating behavior and psychosocial well-being.

Diet and meal timing

Evening chronotypes working shifts reported a cluster of unhealthy eating behaviors that went beyond simple calorie intake. The review found that night owls on shift schedules were more likely to skip breakfast, eat at irregular times, and delay their food intake into late hours. Their dietary composition also suffered, with higher consumption of energy-dense foods, lower fiber intake, and greater alcohol use compared to morning-type and intermediate-type shift workers. These patterns persisted even when controlling for the specific shift worked, suggesting that chronotype influences eating behavior independently of schedule demands.

The concept at play here is chrononutrition, the study of how circadian timing affects food choice, metabolism, and nutrient processing. An evening chronotype working a morning shift must wake well before their body’s natural wake time, disrupting the hormonal and metabolic preparation for food intake. The result is a cascading effect of poor food choices and irregular timing that compounds over time.

Mental health and burnout

The psychosocial findings were equally clear. Evening chronotypes reported significantly higher levels of depression, anxiety, and perceived stress across the 29 studies. Emotional exhaustion and burnout, already elevated risks in professions like nursing and healthcare, were accentuated in workers whose natural circadian preference was mismatched with their work hours. The review noted that sleep disturbances were more common among evening types in shift-working populations, and that these disturbances likely feed back into both emotional distress and eating patterns in a self-reinforcing cycle.

The healthcare sector is a particularly revealing case. Nurses working night shifts who are evening types face a triple burden: a circadian mismatch that disrupts sleep, irregular meal timing that compromises metabolic health, and the emotional strain of a high-stress profession. The review found this combination produced the highest burnout scores across all subgroups analyzed.

The alignment factor

Perhaps the most actionable finding from the review is that the negative effects are not simply a function of shift work itself, they depend on the fit between the worker’s chronotype and the schedule. When a worker’s natural preference aligned with their shift timing, job satisfaction and well-being improved. Misalignment amplified every adverse outcome measured, from poor diet to burnout.

This chronotype-schedule fit concept opens a straightforward intervention pathway: matching workers to shifts that align with their natural circadian preference. For a morning chronotype, that means day shifts. For an evening chronotype, it means afternoon or night shifts. The review suggests that this single organizational adjustment could reduce the health burden of shift work more effectively than many of the individual-level coping strategies workers are currently expected to manage on their own.

What this means for workplaces

The authors propose three practical strategies. The first is chronotype-sensitive scheduling, which requires workplaces to assess workers’ chronotype and assign shifts accordingly at an organizational level rather than leaving individual workers to manage the mismatch. The second is access to healthier food options during night shifts, addressing the nutritional side of the equation. The third is tailored psychosocial support for workers identified as high-risk evening types, including mental health resources and stress management programs.

Each of these interventions is low-cost relative to the downstream healthcare burden of shift-work-related metabolic and mental health conditions, which include type 2 diabetes, cardiovascular disease, depression, and chronic burnout.

Limitations of the review include the predominance of healthcare workers among the study populations, which means the findings may not fully generalize to male-dominated industries like manufacturing or to rotating shift patterns common in factory settings. Most of the included studies were also cross-sectional, which limits the ability to draw causal conclusions about whether evening chronotype directly causes worse outcomes or is simply associated with them.

The bottom line is clear: not all shift workers face the same risks, and expecting them to adapt individually to schedules that fight their biology is both ineffective and costly. Evening chronotype emerges as a specific, measurable vulnerability factor that workplaces could identify with a simple questionnaire and accommodate with relatively straightforward scheduling adjustments, potentially preventing a cascade of diet-related disease, mental health deterioration, and burnout before it takes hold.
